Infant Tylenol Dosage by Gewicht
Acetaminophen (Tylenol) is the most commonly used analgesic and antipyretic in pediatrics. The recommended dose is 10–15 mg/kg per dose, given every 4–6 hours as needed, with a maximum of 5 doses per 24-hour period. Dosing should always be based on the child's weight, not age, as weight-based dosing is more accurate and safer. Infant and children's liquid acetaminophen are now the same concentration (160 mg/5 mL) in the United States and Canada.
Infant Ibuprofen Dosage
Ibuprofen (Advil, Motrin) is dosed at 5–10 mg/kg per dose every 6–8 hours and should only be used in infants 6 months of age and older. Ibuprofen provides both anti-inflammatory and antipyretic effects. Infant and children's ibuprofen liquid is typically 100 mg/5 mL. Ibuprofen should be given with food to reduce GI irritation and should be avoided in dehydrated patients or those with kidney concerns.
Alternating Tylenol and Motrin
For persistent fever in children over 6 months, many pediatricians recommend alternating acetaminophen and ibuprofen every 3 hours. This provides more consistent fever control while staying within safe dosing intervals for each medication. For example: acetaminophen at 12 PM, ibuprofen at 3 PM, acetaminophen at 6 PM. Keep a written log to avoid dosing errors.
Children's Tylenol Dosage by Gewicht
The same weight-based formula (10–15 mg/kg) applies to all age groups. For children over 12 years or over 45 kg, adult dosing (325–650 mg every 4–6 hours, max 3,000–4,000 mg/day) may be appropriate. Always use pediatric formulations for young children — adult extra-strength tablets (500 mg) can easily cause overdose in small children.
🔑 Klinische Hinweise
- Always dose by weight, never by age alone. Children of the same age can have very different weights.
- Acetaminophen overdose is a leading cause of acute liver failure in children. The maximum daily dose is 75 mg/kg/day (not to exceed 4,000 mg/day).
- If overdose is suspected, call Poison Control immediately: 1-800-222-1222 (US).
- Ibuprofen should not be given to infants under 6 months, children with varicella (chickenpox), or dehydrated children.
- Fever itself is not harmful — it is a physiologic response to infection. Treat for comfort, not to normalize temperature.
