What Is Augmentin (Amoxicillin/Clavulanate)?
Augmentin is a combination antibiotic containing amoxicillin (a broad-spectrum aminopenicillin) and clavulanate potassium (a beta-lactamase inhibitor). The addition of clavulanate extends coverage to beta-lactamase-producing organisms including Haemophilus influenzae, Moraxella catarrhalis, and some Staphylococcus aureus strains. It is widely used for acute otitis media, sinusitis, lower respiratory tract infections, urinary tract infections, and skin/soft tissue infections in both adults and children.
Understanding Amoxicillin:Clavulanate Ratios
Different Augmentin formulations contain different ratios of amoxicillin to clavulanate. This is critically important because the clavulanate dose should not exceed approximately 10 mg/kg/day in children to minimize gastrointestinal side effects. The ratios are: 125/31.25 and 250/62.5 suspensions (4:1), 200/28.5 and 400/57 suspensions and chewables (7:1), ES-600 suspension 600/42.9 (14:1), 250/125 tablet (2:1), 500/125 tablet (4:1), 875/125 tablet (7:1), and 1000/62.5 XR tablet (16:1). You cannot freely substitute two 250/125 tablets for one 500/125 tablet — the clavulanate would be doubled.
Pediatric Dosing Principles
For high-dose pediatric regimens (90 mg/kg/day of amoxicillin for otitis media and sinusitis), the ES-600 formulation (14:1 ratio) is essential. Using a standard 4:1 formulation at high amoxicillin doses would deliver excessive clavulanate. Standard-dose regimens (25–45 mg/kg/day) can use the 7:1 formulations (200/28.5 or 400/57). The maximum amoxicillin dose per administration is 2000 mg regardless of weight.
Renal Dose Adjustment
In adults with impaired renal function (CrCl <30 mL/min), the 875/125 mg tablet should be avoided due to the risk of amoxicillin accumulation. Use 500/125 mg every 12 hours instead. Augmentin XR (2000/125 mg) is contraindicated when CrCl is below 30 mL/min. Hemodialysis patients should receive supplemental dosing during and after dialysis sessions.
🔑 Clinical Pearls
- ES-600 (14:1 ratio) was specifically formulated for high-dose amoxicillin regimens in children — it keeps clavulanate within the safe ~6.4 mg/kg/day range even at 90 mg/kg/day amoxicillin.
- Two 250/125 mg tablets ≠ one 500/125 mg tablet — the clavulanate content differs (250 mg vs 125 mg total).
- Always take Augmentin at the start of a meal to maximize absorption of clavulanate and reduce GI intolerance.
- Duration: AOM 10 days (<2 years) or 5–7 days (≥2 years); sinusitis 10–14 days; skin/soft tissue 7–10 days.
- Extended-release (XR) tablets must be swallowed whole — do not crush or chew.
